While most customers use our online request form to request support, we also offer printable forms for those who prefer to send in their knife by mail without submitting a request online.

If you’d like to mail in your knife without using our online portal, please download and complete one of the forms below. Be sure to include the printed and filled-out form in your shipment.
Once completed, include the form in your package and ship your knife to the address provided on the form.
If you use our online service portal, we’ll email you a custom packing slip instead—you won’t need to print a form.

For small, user-replaceable parts, you don’t need to send your knife in. We offer free replacement parts:

A Note on Warranty Coverage
Benchmade’s warranty covers manufacturing defects in materials and workmanship. It does not cover:

Physical or cosmetic damage from misuse or neglect

Blade or handle replacement unless due to a verified factory defect

Modified or disassembled knives
